I, the undersigned, hereby apply to the Board of Directors of Westridge Swim Club Inc., a non-profit corporation, for Membership therein. If this application is approved, I promise and agree, as follows:
1. I shall pay the membership and initiation fee as agreed.
2. To accept such membership, if granted, with the full understanding that the same is subject to any and all restrictions, reservations, and provisions set forth in the Corporate Charter, bylaws, rules, and regulations, and any and all amendments thereto, I agree to faithfully abide by and enforce.
3. As a condition precedent to the use of the corporate facilities, to pay in advance the annual membership fee to help defray the costs of maintaining, operating, and improving said facilities, which annual fee shall be determined from year to year by the Board of Directors. Annual dues for each year of operation shall be due on or before a date determined by the board.
4. It is specifically understood and agreed that if I fail to pay any remaining balance due on the membership or dues by the due date, Westridge Swim Club, Inc., shall have a first lien for all sums due upon said membership or dues.
Submitting this form is the equivalent of an electronic signature, and will be treated as such.
